The Case Management program assists and advocates for people living with HIV/AIDS, to
overcome barriers and become connected to needed community services. Guided by harm
reduction philosophy, we offer client-centered and easily accessible case management services
to support people in reaching their own goals by providing:
- Daily walk-in case management from 10 AM to 4 PM, Monday through Friday
- Information and referrals to community-based services
- Emergency food vouchers, nutritional supplements, nutritional education/information,
and information on food assistance programs through our Nutrition Program
- Supportive services and support groups (support groups include: Gay and
Bi Men’s Group, Haitian Group, Sub-Saharan African Group & Women’s Group)
- Multicultural and Multilingual Case Management Services. Languages spoken included: Spanish, French, & Haitian Creole.
- Home visits and appointment accompaniment if needed
