Case Manager: Haitian and Sub-Saharan African Populations



CCA is seeking a Case Manager
for its Client Services Department.

Cambridge Cares About AIDS seeks an experienced case manager to work at the Case Management Program. This program assists and advocates for people living with HIV/AIDS to overcome barriers and become connected to needed community services.

The mission of Cambridge Cares About AIDS (CCA) is to respond to the HIV/AIDS epidemic by serving those most impacted by social and economic inequality. CCA provides accessible prevention, education, advocacy and support services in accordance with harm reduction principles to help people meet self-identified goals.
 

Position Description

The Case Manager will provide case management services to approximately 50 people living with HIV, with an emphasis on providing services to the Haitian and Sub-Saharan African populations. This position will coordinate and facilitate the Haitian support group. This position will develop appropriate service plans and facilitate access to services such as immigration, medical, housing, financial, and legal services for people living with HIV. The case manager will provide clients with easily accessible and low threshold services that operate within the harm reduction framework, which includes assisting clients with accessing support services on triage, walk-in, and as-needed basis.


Qualifications
  • BA or equivalent, two years human services and/or case management experience.
  • Bi-lingual in Haitian Creole.
  • Bi-lingual in French, Portuguese, or Spanish a plus.
  • Experience with the provision of HIV services.
  • Experience working with issues related to substance use, harm reduction, homelessness, mental health, and bi-lingual/bi-cultural communities.

Salary Range: High 20’s to low 30’s
